REVIEW THE FOLLOWING INFORMATION
If you responded YES to any of the above questions, access to campus is not authorized unless (1) comply with the following standards (based on the CDC’s guidelines for Discontinuation of Home Isolation and When to Quarantine), which may require quarantine or isolation for a period of time, and (2) have been approved to return to campus by the Office of Human Resources.
Exposure and quarantine:
- •Anyone with a potential exposure to COVID-19 must not visit campus for 14 days after the date of the last potential exposure. Stay home for 14 days after your last contact with a person who has COVID-19. Watch for fever (100.4°F), cough, shortness of breath, or other symptoms of COVID-19. Stay away from others, especially people who are at higher risk of getting very sick from COVID-19.
- •Anyone with a potential exposure to COVID-19 is recommended to receive viral testing approximately 3-5 days post-exposure.
- •If a person tests negative for COVID-19 after a potential exposure, it means that the person may not have had COVID-19 at the time of testing. A negative test result does not eliminate the possibility that the exposed person could develop COVID-19 during the remainder of the 14-day virus incubation period.
Options to reduce quarantine:
- •Quarantine can end after Day 10 without testing and if no symptoms have been reported during daily monitoring.
- •When diagnostic testing resources are sufficient and available, then quarantine can end after Day 7 if a diagnostic specimen tests negative and if no symptoms were reported during daily monitoring. The specimen may be collected and tested within 48 hours before the time of planned quarantine discontinuation (e.g., in anticipation of testing delays), but quarantine cannot be discontinued earlier than after Day 7.
Quarantine for Vaccinated Persons:
Vaccinated persons with an exposure to someone with suspected or confirmed COVID-19 are not required to quarantine if they meet all of the following criteria:
- •Are fully vaccinated (i.e., ≥2 weeks following receipt of the second dose in a 2-dose series, or ≥2 weeks following receipt of one dose of a single-dose vaccine),
- •Are within 3 months following receipt of the last dose in the series, and,
- •Have remained asymptomatic since the current COVID-19 exposure.
Vaccinated persons who do not meet all 3 of the above criteria should continue to follow current quarantine guidance after exposure to someone with suspected or confirmed COVID-19 set forth above.
COVID-19 illness and isolation:
- •Anyone with COVID-19 signs or symptoms must not visit campus until they have had no fever for at least 24 hours (without the use of fever reducing medicine), the other symptoms have improved, and at least 10 days have passed since the symptoms first appeared.
- •Anyone who has tested positive for COVID-19 but does not have any symptoms must not visit campus until at least 10 days have passed since the date of the first positive test and they continue to have no symptoms since the test. For 3 more days, the individual must stay at least 6 feet away from others and wear a face mask covering their nose and mouth when other people are present.
- •Anyone who has tested positive for COVID-19 AND had any symptoms of illness must not visit campus until at least one day has passed without a fever (without the use of fever reducing medicine) and symptoms have improved and at least 10 days have passed since symptoms first appeared.
- •Anyone who has tested negative for COVID-19 AND had any symptoms of illness must consult with their healthcare provider for return to campus recommendations. This documentation must be provided to the appropriate University representative, and approved, prior to the student or staff member returning to on-campus activities.
- •If, due to the individual’s particular circumstances, other procedures related to the discontinuation of home isolation are recommended by the individual’s health care provider, a local public health official, or the CDC (e.g. a longer home isolation, testing prior to discontinuation of home isolation, etc.), those recommendations must be followed.
If you responded NO to all of the above questions (or if you responded YES to any question and you meet the conditions for authorized access set forth above, as applicable), please read and submit the certification below.

By submitting this questionnaire below, you certify that your responses to the above questions are true, you meet the all of the conditions for authorized access to campus set forth above (as applicable), and that you will comply with the precautions for preventing the spread of COVID-19 as required by the University, including the following:
- •Do not visit the University's campus if you have (or recently had) signs or symptoms, a diagnosis, or potential exposure to COVID-19 unless you meet the above criteria for authorized access to campus.
- •Wear a cloth face covering or a disposable, multi-use mask and keep a social distance of at least 6 feet at all times while on campus.
- •Wash your hands frequently for at least 20 seconds, especially after coughing, sneezing, using the bathroom, or touching items in a public space. Use hand sanitizer if soap and water is not available.
- •Avoid touching your face. Also avoid touching all high-touch surfaces. If this cannot be avoided, wash your hands immediately afterward.
- •The University requires compliance with all COVID-19 precautions recommended by the CDC or required by local government/public health officials. COVID-19 precautions must be taken at all times while on campus no matter how many people are on campus.
- •Visitors who experience signs or symptoms of COVID-19 while on campus must report that fact to Police Chief Kevin Koo at 513-244-4226.
- •Employees who have signs or symptoms, a diagnosis, or a potential exposure to COVID-19 must report that fact to Director of Office of Human Resources Lisa Kobman at 513-244-4979.
- •Students who have signs or symptoms, a diagnosis, or a potential exposure to COVID-19 must report that fact to report to Health Services Manager Amy Demko at 244-4408.
