Title Description

If you should become ill or are exposed to COVID-19, you are required to report those symptoms to Health Services and/or Human Resources. This form is for students and employees and will allow you to submit your report to the appropriate office; you must be logged in to myMount to access and submit the form.

For students or employees who need to submit a daily screening questionnaire; you must be logged in to myMount to access and submit the form. Weekly COVID-19 Surveillance Testing occurs each Wednesday during the Spring semester from 2-5 pm in the Harrington Center Food Court. Click this link to schedule an appointment. Note: This testing is for individuals who are feeling healthy and well only. Individuals with symptoms of COVID-19 should complete the Mandatory Reporting Form and NOT come to campus.

For Fall 2021, this policy is applicable to all students residing in the Seton Residence Hall.
