If you should become ill or are exposed to COVID-19, when your vaccine is not up-to-date or you are unvaccinated, you are required to report those symptoms to Health Services and/or Human Resources. This form is for students and employees and will allow you to submit your report to the appropriate office; you must be logged in to myMount to access and submit the form.
