You can also view these procedures as a Word document instead of using this interactive version.

Note: Retain all records regarding contact tracing, including your notes, in a secure location separate from non-medical, non-confidential files.
Resource A - POSITIVE TEST
Items to discuss when calling a person to notify or confirm the positive test:
Note: Individuals are “potentially contagious” beginning two days before symptoms began or before their positive test if asymptomatic. “Close contact” is within 6 feet for a total of 15 minutes or more in any 24-hour period.
A form of this email will be sent to a student or employee after phone contact is made:
This email will confirm our phone call earlier today regarding your positive COVID-19 test.
Please begin quarantining immediately. You may not return to campus until INSERT DATE OR OTHER RETURN REQUIREMENTS BASED ON THE SITUATION. You should also review the University’s COVID-19 Safety Guidelines for information about when you may return to campus and other important resources and procedures.
If you are a student, review the Student COVID-19 Absence Policy and contact the Wellness Center if you need a Class Note. Employees should contact their supervisors and follow all applicable attendance/absence reporting procedures.
We will commence contact tracing for members of our campus community based on information from you. We appreciate you identifying any student, employee or campus visitor who you may have had close contact of at least 6 feet with this individual for approximately 15 minutes or more in any 24-hour period while you have been contagious.
You are encouraged to contact your healthcare provider, to monitor your symptoms daily and report your situation to the Wellness Center or the Office of Human Resources, as applicable, if your situation changes or if you need any additional information.
Thank you for your continued adherence to the University’s COVID-19 Guidelines. Please feel free to reach out to me with any questions. We look forward to seeing you back on campus again soon.
